Left Click on the Editable Area, of the location on the page, that you want to add your form to.

Question: Type the Question you would like to Ask in the text box.
Answer Type: Choose how people will be allowed to answer. If you want a free-form answer, pick a text field (for shorter answers) or a text area (for paragraph-style answers). If you want them to only choose one answer, choose radio buttons. For a drop down, choose select box. For a text list where they can choose multiple options, choose list box.
When you choose your answer type, a new field will appear. Answer Options: These are the answers that your visitors can select. For questions with multiple possible answers, put each answer on a separate line.
This question is required: Check the box if you want to Require that this Question be... Answered If the form is submitted without this CHECKED Question being answered. The form produces an ERROR Message asking that the Question be Answered before going further.
Preview... Tab Selected
Preview: Allows you to look at the Form you have created before you Update it or Publish it. This means less steps to get it... Just the way you want it.
As you can see in the image above, the TWO lines highlighted yellow, are NOT line breaking in the best spot. The text generator wants to fill the first line completely up before starting the next line. But... in this case since there is going to be TWO lines, its best to line break between the two thoughts expressed. So Next... Lets Select the Edit Tab... To edit what we have and create a better Question layout.

As you can see highlighted yellow in the image above, We go to Question Text Box and insert the HTML for Line Break... <br> Right in between the two thoughts expressed with No Spaces needed.
To create... HTML Select à Add to (Any Edit Page Area) Select à Add New Block... Content Create EXACTLY what you want HOW you want it. Select àHTML located in the 2nd line of the tool bar, second icon from the right. This will change the display to show the HTML that controls the text layout. Simply... Highlight the Text and HTML - Then Copy it - Then Close your way out Open your way back into the Question text Box and Paste what you copied into it.

Now that you have more than one Question loaded, you can see that each question has an Up and Down Arrow. You can reorganize the questions or edit/delete them once they're added.
When you're satisfied with how the form is arranged, click... Add
Options... Tab Selected
Form Name: Name the form with something that will remind you about the form, what it contains and that will distinguish it in the dashboard form results area.
Message to display when completed: When the Form is Submitted... This is the Message on the next screen, Thanking the Submitter and letting them know... The Form has been successfully submitted.
Notify me by email when people submit this Form: Toggle this... On - If you want an email notification every time a Form is Submitted.
Solving a CAPTCHA Required to Post: Toggle this... On - If you want to require that a CAPTCHA be correctly filled in before the Form can be Submitted. With this feature activated you will NOT receive computer generated junk form Submissions.
When you're done, click... Add
Exit Edit Mode
If you're satisfied with everything, click... Exit Edit Mode - To save your changes and publish live, or preview for your self.
